The Legacy Planning Kit includes information on ways to make a gift at death and documentation for your executor, including a Catholic Durable Power of Attorney, a Catholic Declaration on Life and Natural Death, Information for Your Executors, and Obituary Information. For our pastor and the parish office, the kit provides a Catholic Legacy Gift Notification Form, detailed wake or vigil liturgy, and funeral liturgy, as well as the graveside service.
